Mentor application form
Mentors participating in the mentoring sessions provide a form explaining their interest and contribution in the program, as well as their experience and knowledge on the mentoring field.

Mentee application form
Mentees participating in the mentoring sessions provide a form explaining why they are interested in taking the mentoring sessions and the reasons they want to start their e-business. A brief description of their business idea and expectations from the mentoring program is provided in order to help the project partnership to match them with their WIN on-line mentors.

Mentor Cooperation Declaration
This is a Declaration form, addressed to mentors who want to participate in the mentoring sessions. It is declaring a signed collaboration between a mentor and the responsible organisation in each project country and consent.

Mentee Cooperation Declaration
This is a Declaration form, addressed to mentees who want to participate in the mentoring sessions. It is declaring a signed collaboration between a mentee and the responsible organisation in each project country and consent.

Mentoring log
The Mentoring Log refers to a mentor-mentee diary, providing a detailed analysis on the topics discussed during each mentoring session. Mentors must complete this document, explaining the method they approach to contact the mentees, the session’s accomplishments, suggestions and other relevant comments.

Mentor self-reflection Diary
This is a self-reflection diary, completed by mentors on a regular basis to provide detailed information on the topics discussed during each session. For example, what went easily or hard, what they learned about themselves during the meeting with mentees, and provide relevant notes and comments.

Mentee self-reflection Diary
This is a self-reflection diary, completed by mentees on a regular basis to provide detailed information on the topics discussed during each session. For example, what went easily or hard, what they learned about themselves during the meeting with their mentor, and provide relevant notes and comments.

Mentoring Code of Ethics
This document reflects the Code of Ethics introduced for the smooth and effective implementation of the mentoring sessions, focusing on fundamental rules, proactivity, motivation and availability of both groups participating in the mentoring program, their impartiality and objectiveness, professional confidentiality, responsibility and respect, as well as know-how development.

Final Evaluation Questionnaire for Mentees
Mentees, after the completion of the mentoring sessions, have to provide the partnership with a questionnaire which is distributed to them for evaluating their mentors’ contribution and work. The mentoring process and the relationship with their mentors must also be evaluated, the effectiveness of the mentoring meetings, the lessons learned and the skills and knowledge gained from the mentoring program, the organisation process defined by the responsible organisation in each project country and recommendations must be shared to the project consortium.
